Registered Nurse (RN) – Scranton / Dickson City, PA
Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to deliver high-quality, patient-centered care. In this role, you will conduct assessments, manage individualized care plans, and provide direct skilled nursing care in coordination with an interdisciplinary team.
Key Responsibilities:
Direct Clinical Care: Perform comprehensive physical, emotional, and psychosocial assessments. Deliver skilled interventions including wound care, IV therapy, medication administration, and symptom/pain management (hospice/palliative care).
Care Planning & Coordination: Develop, implement, and evaluate individualized care plans in collaboration with physicians, therapists, social workers, and care aides. Respond promptly to changes in patient condition.
Patient & Family Support: Educate patients and caregivers on care management, disease progression, and health maintenance while offering emotional support.
Team Supervision: Oversee and delegate tasks to LPNs/LVNs, Home Health Aides (HHAs), and nursing support personnel.
On-Call & Emergency Response: Participate in scheduled on-call rotations, patient admissions, and urgent clinical responses as needed.
Documentation & Compliance: Maintain accurate, timely clinical records in full compliance with state/federal regulations, agency policies, and professional standards.
Qualifications & Requirements:
Education: Graduate of an accredited School of Nursing (ASN or BSN).
Experience: Minimum of one (1) year of clinical RN experience required (Home Health or Hospice experience preferred).
Licensure: Current, unencumbered Registered Nurse (RN) license in Pennsylvania.
Certifications: Active CPR/BLS certification required.
Driver Requirements: Valid driver’s license with an insured, reliable vehicle and willingness to travel within the service area.
